How to add name in ration card online

  • To add name online in the ration card, first of all visit the official website of Uttar Pradesh Supply Food and Logistics Department fcs.up.gov.in.
  • After this the home page of the website will open on the screen then go to the login page.
  • If you are already registered on the portal, then enter the login ID and password, if not registered then create the login ID and password, and then login.
  • After login, click on the option to add name of family members.
  • Now the application form will open in which fill the details.
  • To add the name of the member in the ration card, you need a birth certificate, residential certificate or marriage invitation card.
  • The complete list of documents is given in the website, you can upload your documents from there.
  • Then click submit the application form.
  • After that you will get an acknowledgment number by which you can check the status of your application.
  • After 1 month, your ration card will be delivered home by post.

How to add name in ration card Ofline

  • First of all you have to go to the food supply department of your district and from there you have to get the form.
  • Now you will have to enter all the information asked in the form correctly, as well as the relationship with the new member and the reason for giving preference in the ration card.
  • After that attach all the documents and submit the application form to the office. And pay the application fee as applicable.
  • After that you will get an acknowledgment number from which you can track the application status of ration card.
  • Office workers will issue your ration card after 2 weeks after document verification.
